In Medical Care, Nova Fellow Heidi Gullet and Scholar Kurt Stange explore a scalable electronic health record (EHR)-facilitated, clinic-to-community linkage (CCL) program that addresses social needs between health systems.
This study looked at whether primary care clinics could use electronic health records to connect patients with community help for things like food, housing, and exercise. Six clinics referred more than 1,200 patients with chronic health issues or asthma to United Way 211, a service that linked patients to local resources. Of the patients reached, nearly all had at least one social need, and most had at least one need resolved or in progress. Although some patients saw improved blood pressure, other clinics not using the program improved just as much or more.
The study shows this approach is feasible, but more research is needed to understand its longterm impact and how well it works compared to other options.
